This is a not-so-subtle warning that if a couple's romantic relationship should ever end, the male subject of the song will surely die of grief, and therefore the girl would do well to take out some life insurance on him. It's an expression of emotional dependence verging on emotional blackmail, although almost certainly meant to be taken hyperbolically, rather than literally.
