I always thought the "ghost" was a demon, but was actually just a metaphor for schizophrenia. I remember reading somewhere that Akerfeldt's mom was a schizo, which lines up very well with the lyrics

I think the song is about someone fearing their inner demons, being afraid they've sinned and will go to hell. They are afraid of the unknown, hence the line "Rode into the dark unaware." I am not sure if this is accurate or not, but to me it sounds like the song is about fear in general- fear of death, fear of "god", fear of what's to come.
It could also be about a ruined soul, but I'm just stabbing in the dark here. By a ruined soul, I mean someone being possessed and having their soul stained by the unholiness.
