A guy is on his deathbed, his girlfriend is so shaken up at his upcoming passing, he feels the need to console her and apologize.

This song is talking about the need for human equality and world change, not about a guy and a girl breaking up. That is too fucking cliché. Every site have seen I see the same ignorant interpretations.
In my opinion this is what it means.
“These colors will not change. You change the way I see them”. This line references a change in perspective
“These words will fade” is a reference to the frailty of the human condition. That is, we aren’t immortal, neither are our unique perspectives.
“Can you explain why you hate them? We are the same”, just points out that everyone; no matter the perspective, nationality, or religion; we are all HUMAN and need to empathize with one another and not hurt each other over petty differences.
“She keeps repeating, all that she needed. She said she’s right here. She seems so distant”, alludes to mother nature, or earth, and how she shows us that the ways we need to take care of her, but as modern humans we are increasingly trying to separate ourselves from nature through indulging in the superficial.
“Saying goodbye, this time, the same old story. Seeing you cry makes me feel like saying sorry”. This chorus points out the nonchalantness in which we carry on our day to day lives in, and that we realize what we are doing to the nature and to one another as humans on a world scale, but don’t really care that much.
“Just a few last hours. We gotta make this count”, displays that we are running out of time to correct our mistakes. “We are counting backwards…… We gotta make this count, we are falling forwards” refers to the fact that we are going about it all wrong . We are looking to the past (looking backwards) and not paying attention to where we are headed (falling forward), but all the while we think we are doing the right thing. -
