I believe its about split personality. Corey talking about his role as stone sour's lead singer and slipknots lead singer. Slipknot being the more darker one and stone sour more reasonable.

It is about a person being lost, trying to get help but there is "nobody out there", so in order to survive, he has to do "what must be done" which could be a range of normally non-human activity - at the expense of one side of a dualistic reality. "You would have done it too" means there wasn't any choice in the matter, and to me relates to not having the luxury we are so used to...
