Think it's about suicide (obviously) but also abortion. Dead inside is like being killed inside the mother's womb. Cyanide is also used in abortions to kill the fetus.

This song might be a link to The Peoples Temple cult who performed a mass suicide in 1978 using cyanide, killing 918 people..

As many MetallicA songs, Cyanide has multiple meanings
My 2 favourites are these:
1) as the oldest comment said, it was about WWII, gas chambers, and nazis. The demo was entitled German Soup for that reason.
2) Drugs. Since Metallica started to play many fellow singers, musicians and well-known people have died of drug abuse. The cyanide are drugs, so, as Metallica has survived, they are asking for permission to Death, so it may let them outlive all of their contemporary mates. It's all about old wasted rockers -
I believe it has to do with the gas chamber death penalty in the united states, because cyanide is used in that form of execution.
