It's about being a sane person in the world of the insane. Being chased by them.

God this song hits me like a shot of heroin. It is about physchosis, or hallucinations, mania or whatever, but the lyrics go way beyond the simple object of madness, of one who is unwell...they show you what it's like to live a different, immensly intense reality than most people. And also, it shows that he recognizes his "monsters" and has been barely holdiog on to this, tactile sanity- but imo it shows that he wants to become stronger and say FUCK YOU, YOU GODDAMN PHANTOM OF MY INNER BEING...and start to take control of himSELF and not allow his demons to destroy him. I could have written this exact song, today. Fucking mind games, aka my new diagnosis: schizoaffective disorder. I haven't told many people becuz its shameful and makes me weak. But not for long. Screw mind tricks. Sorry for going on, Metallica is effing freakishly amazing. I've just discovered this.
