It was about two things, Sean and the Beatles. The lyric "when did we lose the touch, that use to mean so much etc." is talking about John's relationship with mainly Paul but also George and Ringo.At the time when John wrote this song he head been visiting Paul often and was thinking of writing songs with him again. But it did not happen because Yoko told him no.

This song is indeed about Sean Lennon. John was up late one night and couldn't sleep so he took Sean in his arms when he was just a baby and began to write these lyrics. He recorded an extremely raw version of this song a few years before he died but it was never released. Then in 1996 Yoko brought that early recording to Paul and he got George and Ringo to come together and make it a presentable song. George and Paul also sang backup vocals to strengthen the sound of John's voice. This is truly a beautiful song and just goes to show you why John Lennon is in fact the greatest lyricist of all time...along with Paul McCartney.
