This song was inspired by a true story- some young men from Long Island (home of Blue Oyster Cult) that Donald "Buck Dharma" Roeser knew were murdered by one of their partners while driving down to Mexico to buy a stash of drugs that they hoped to bring home and sell for a lot of money.
The song tells the story of these young men and their violent deaths. But Blue Oyster Cult gave the song a horrible twist by revealing at the very end that the person singing, the person telling the story, was the driver who had cold-bloodedly killed those young men.
