I think what most people are missing here is that Dylan was a revolutionary, this song doesn't mean that he is only "love sick" in the literal sense, but he is also sick of all the problems in his life. When he says "I'm walking through the streets that are dead" what he really means is that he is the street. He is tired of being walked on, by his fans, the media, and other artists. He feels dead from all this pressure. Mix in the fact that this is his last album, and you can see that he just wants to be free.
