    I think he is looking up in the sky seeing one moon but since he is drunk he sees two moons and he mentions "this ain't good for my health" and "sorry please excuse me for my mess" as in he is talking to the people at the bar then he says "my heart has been pouring through my chest" means he had a breakup and went to the bar to drink.

    If he sees two moons and soon after says this ain't good for my health its most likely to say he is drunk
