I do believe this is from Richtofen's POV I mean it makes so much sence, Maybe he's just dreaming all of this. Pherhaps he's refereng to the voices in his head. Maybe he hears Samantha's voice in his head or maybe Maxis'

maybe he's refering to the test subjects and their progress
Richtofen might see things other's cant see.
"You dony cry when I am gone" I think he's expecting his death and tells the test subjects to continue the fight even after his death.

Maybe he has something incomin with either Samantha or the voices in his head where both of them have hunger for killing.

Richtofen and the surivers fight everyday and he doesnt want to give in till he get's what he wants because it will break everything he stands for if he does give in, he knows he's a hated and despised person but is okay with it cuz he know's he's also powerful and can do anything.

I do agree somewhat with that it is from sam's point of view, but I also have a feeling it's from Richtofen's. When it's saying "all inside my head" I think it is referring to Richtofen's insanity and that he hears things and sees things in his head. "Dead is magic" may be referring to his love of death/killing. "it's killing me to see I'm killing you" may also be referring to when he experimented on the other survivors and they are slowly dying. "Don't cry when I am gone" may be telling the other survivors not to worry if he dies, or maybe that they won't cry because they seem to hate him. That's all I have but it's anyone's guess what the real meaning is!

Samantha Maxis IS dead, but she lives through the zombies."These shackles they come/With certain opportunities/They won't let me get away/But they show me we're the same" pretty much says that she also controls them, but she cannot get away from them, and "They're all inside my head" says the same thing. maybe she needs the 115 to make herself a new body so that she can get away from them. the guttural voice could be the zombies speaking through her to the survivors.

I think this song is from, and this may sound very weird but a zombies point of view. If not that than Samantha's point of view.About if it were Samantha's point of view , the lyrics with the mos sense of that must be "I can fly like a bird not in the sky" an "I follow and lead as you pass". It is like Samantha is following her father, but leading the zombies. "I don't feel like I am real, but I know it's all imaginary. Are they real and do they feel is all the pain just inside my head". I believe this song as much to do with the zombies story line, and is a very sick song for those who have not heard it.....
