The lyric says she's lying back. l believe that implies she's doing nothing but watching.So the dude is doing something to himself that in someway hurts either physically and, or emotionally. Perhaps an S&M relationship? This concept is strengthened when the line, I think it's a shame when l get feeling better when I'm feeling no pain, is added. As far as the other girl, who he seems to care about, wears faded jeans. He warns Sundown to beware of being caught. rla
