The lyric says she's lying back. l believe that implies she's doing nothing but watching.So the dude is doing something to himself that in someway hurts either physically and, or emotionally. Perhaps an S&M relationship? This concept is strengthened when the line, I think it's a shame when l get feeling better when I'm feeling no pain, is added.
