He's fallen out of love with her(please don't hate me then/I could not pretend) even so, he feels remorse and guilt for hurting her. He imagines her crying (I can feel the tears/running through the years)

I think the song is pretty clear - it's about a man breaking up with his wife or girlfriend because of changes within him.
The tone and words make this one of the saddest songs I've ever heard.
