It seems like a kind of break up situation within a relationship and the situation is being relived in memory where the outcome can't be changed no matter how hard she tries to punch out of it she realises its impossible which then breathes fire into the nightmare and its tearing her apart. Quite possibly she lives in a town that's small in which she sees the person she was once with so memories come back, hence the part where the guy sings

Its similar to a situation I've been in and has been a struggle to get out of. -
My opinion is that it's about the struggles and influences (like drugs) of fame and how it's harder to deal with than people think it is. It's hard to get away from the glare of the public when suddenly, everybody knows every little mistake you've made in the past. I think she's trying to avoid the negativity of all the attention, but she thinks that performing and making a name for herself is worth it.

No I think the above poster is quite correct. I first thought "punching in" was like dialing in or up something but it's clearly the inability to punch something or someone when dreaming. I also have had great difficulty in dreams trying to strike or punch someone, all of a sudden an impossible effort is needed to continue to swing or it's like there is a force repulsing my arm or dragging it back, like trying to punch something underwater but worse. Frustration & inability

In my dreams, I have never been able to punch. Everytime I try to punch someone in a dream, it feels like two south ends of a magnetic pole bouncing off each other. I can't do it. So when I first heard this song, I wondered if this happened in others dreams, and if that was what "Punching in a Dream" referred to. And if you read the rest of the lyrics, this really makes sense and fits into the overall theme of the song. Not being able to punch in a dream is a very frightening thing, feeding into the line "breathing life into a nightmare." The song has alot to do with fear and falling apart, and my theory of not being able to punch in dreams seems to fit, but song lyrics are always what they need to mean for a specific person I guess.
