I think this song is about lusting for another woman, the guilt on infidelity and the pain of desire. She is portaied as Lilith as it is a demonic, impure feeling of lust and nothing else. That poison is the raw desire that kills his insides while he is trying to resist those urges, hence the "i never asked for this, i never wanted this, i never asked for you, i never wanted you" but he cannot fight back "throws me in your arms again"

Lilith is also a biblical character (Adam's first wife) that murders pregnant women and infants. She was banished because she would not submit to Adam. She also seduces men. I think it's a reference and he calls her Lilith as an insult. Most likely because she is evil like the biblical character.
