Love is very hard, but he must make the climb. Here, you have a metaphor where love is compared to climbing a mountain. If you steadily climb a mountain, you can reach the summit, but it's very hard. He talks as if if feels the world on his shoulder. The world symbolizes intense pressure someone might feel. The world on his shoulder symbolizes the stress he feels in his life.
Then, he says through the clouds he sees love shine.
