I lived on the streets for a bit and this song always reminded of Methadone maintenance crowd. They only had to be somewhere twice a day and the rest of the day they would just hang around the park without care in the world...

I think it is simply a sarcastic song about someone waiting for a lover who stands them up.

It's a song about his love for heroin. But he knows that is all just a skin wrapped around misery.
