I think it’s about cellular disease. It’s a riddle where you would almost assume it’s about a person but no person doesn’t bleed, so it looks like the riddle is about a cancer cell. And cells make up a whole person which is why it’s posed as if a person. It’s from BLEACH which is titled by how you get rid of a stain to point out the double jeopardy of an initial problem, and its solution making it worse in a more complex way. Chemicals like that found in bleach, and other harsh products, when injested are known to cause cancer. The cell could be a red blood turned into a cancer cell. A cancer cell by something foreign like chemicals put into the body is metaphorically like a stain.

The song is probably about bad role models in the society, "I'm a stain" actually speaks for itself. "He" does nothing,and that means that his behavior shouldn't be emulated by others. It could also be about people who don't fit in society.
