The patient is dying and the doctor is schizophrenic, the patient is talking about their struggles while they live together. That's what I got out of this song.

I always saw it as a man who is dying of cancer going over the life of his girlfriend/wife. She's upset that he's dying and he's singing all the reasons why she's messed up, but also why she shouldn't worry.
