I think it is about a woman who was involved with someone who is the lover of one of her friends (perhaps a one night stand). Perhaps a pregnancy resulted. She chooses to keep the event to herself because she treasures the friendship she has in spite of the deceit by the guy who betrayed both of them. At the same time, those around her know there is something going on, and keep trying to get her to talk about it, for the sake of gossip. The song is saying that her silence is understood and respected.

It's likely about a woman who's been sexually assaulted and can't talk about it. She chose her option to live with the rage, anger, and emotions she has inside of her instead of talking with someone to let it out. She can't talk about it because it would involve reliving the event. She believes she can bury her feelings so deep inside that they will never come out. There never seems to be a good time to bring it up so she tries to deal with it in a different way, hoping no one will know what happened to her. She may try to push away those closest to her who deeply love her and may sense she is hurting. She may not realize how much it is affecting her life and that sometimes people around her, while not knowing how to respond, can see the rage and anger she has just under the surface. She also may not see that there are those close to her who love her and would want her to know that she did nothing wrong. It was not her fault in any way. They would want her to know that if she chose to confront her feelings, at the risk of possibly reliving that event in part, she would be supported by people who offer unconditional love to her if she chose to accept it. To anyone who understands this meaning please consider sharing your feelings with someone close or a professional who will walk with you to allow you to confront and release those feelings. Let loved ones pushed away back into your life to support you. It was not your fault, you are so loved and respected. You should not have to carry this burden. You did nothing wrong, it is not your burden to carry.

As a Billy Joel junkie, I believe Code of Silence is about a breakup. "You don't wanna lose the friendship.." It is very obvious.
