It seems to me that this song is, as the refrain suggests, about the music and dreams. It reminds me of the story of artists such as Bruce Springsteen, who almost failed before he became a star. This song seems to suggest that, in the end, all successful artists are dreamers and it took a lot of determination and heartbreak for them to make that dream come true. In light of the music video, "Keep on Dreaming Even If It Breaks Your Heart" was a song by the old man with the "beat up old guitar." He inspired the band playing the song, who in turn passed on the Music to the next generation (the kids and teenagers they each give their instrument to). It is these kids who, in turn, inspire the Old Man to keep his guitar and his music.
