For me, it's about loss, specifically about the loss of a child/baby. Especially the line of "cuts me with every could have been" could have been a girl, could have watched her grow up... "Clambering for scraps in the shatter of us collapsed" refers to trying to salvage their relationship as the loss of the baby shatters the world around them.

I think the question might be much larger than a break up. I think the song is about loss of any kind and th wretched feelings that one fells until time heals everything.

A breakup has just happened and she is confused and lost as to what happened and how to handle it.

shes trying to put the pieces together of what happened to cause the break up. picking up the pieces of the broken glass so to speak to try to figure how it broke.
"It cuts me with every could have been"
It hurts more and more as she figures it out
the broken scraps shes picked up are cutting her.
"Pain on pain on play repeating."
She keeps replaying it over in her head
"Everybody says time heals everything but what of the wreched hollow? the endless in between? are we just going to wait it out?"
She feels the wounds will heal eventually but what until then?
