It's self explanatory when you just look at the lyrics on the surface level on wordplay. You have Javier sort of confessing to this unknown entity and is basically trying to expose himself about his "frayed" heart, putting himself in a position where he's vulnerable and is able to just put everything on the table. He's in a great deal of pain (obviously in spirit) Because of some sort of past history or whatever it is to get him to be so heartbroken. But I think is willing to just let go of everything and throws everything out there to try and ask for forgiveness.
It's sort of this unconditional love whether he feels skeptical about it all or what but he has faith in this person or God that has hurt him and wants them to put him back together piece by piece!
"don't ever let me let it go" - great line if you ask me -
At this moment I'm down the hill and when I first heard this song it uplifted me in a way that only God could do. Literally I felt my strength back and I know that the pain I felt will be healed by His Grace stitch by stitch.

This guy has obviously Been through some heartbreak so he seeks out either another woman or he could be talking about turning his faith back to God. Considering how Slowly but surely God can heal someone physically and emotionally, through a little bit of faith and not "falling" and not letting go of it.

The guy was afraid of making a commitment, so he runs off to another who used and abused him. Now he returns to the woman he walked away from with heart in hand. He discloses to her the sufferings of his wounded blue and black heart. Having been (there) with another and (now, back to a continuation of physical meaningless romance he calls medicine, and healing. He's giving girl#1 all his pieces broken asking her to put his torn-up heart back together now stitch by stitch or,(carefully without words). You see he was afraid of falling in love and still is: because hes'been seriously wounded by others; but willing to give it a try, he says, Don't ever let me let it go. I've been (there and) now I'm (back) to you with frayed heart and open scars, because your touch makes me whole again.
