i feel like its how you can be at a constant war with yourself that can make you feel a certain way

I think the song represents disorders that are built upon social phobia etc, and fear of judgement, abandonment and embarrassment. And things like these are so overwhelming that they end up struggling with voices either from things like schizophrenia (or anything including psychosis- voices only existing in the victims head) or thoughts that are self hate etc. And either one or both of these bring them under the surface separating sadness (underneath) and happiness (above).

A person struggling with DID, OSDD or something like them, not believing they would be accepted.

To me, the song represents someone fighting an internal battle that they know they won't or can't win, at least not right now.
