Kanye had reconstructive surgery on his jaw after a near fatal car crash. The song is about the crash and how he is rapping ‘through the wire’ put on his jaw in the surgery. The ‘hospital where biggie smalls died’ is actually the hospital he was admitted to which is where biggie passed. ‘Unbreakable, what you thought they’d call me Mr. Glass’ refers to how he survived his crash whilst referring to the antagonist Mr. Glass from M. Night Shyamalan’s film ‘Unbreakable’
