He lives inside my mouth
And tells me what to say
When he turns the trains
On he makes it go away
The hands are cracked and dirty and
The nails are beetle wings
When he turns the trains on he
Unties all of the strings
The worm: "Tell me something...

I think this song is about Marilyn Manson and what he is becoming, Antichrist Superstar is the ending of the trilogy, so it takes place in the future, Marilyn Manson has always struggled with split personality disorder, in the story Marilyn Manson is the worm snake that is wanting his wings so he can become the Angel Prince Brian Warner again, He's looking for his innocence, but the longer it takes him to unmasked himself destroy the character Marilyn Manson the older he's getting, his hands are cracked and dirty and his nails are beetle wings, at the end of the story he gets his wings but the boy that Marilyn Manson loved is THE MAN MONSTER THAT HE FEARS, in his mind he is his grandfather Jack Warner, oh no it is everything he said it was, oh no I am all the things he said I was, this is what you should fear, you are what you should fear

At least part of this is about how Reverand Manson viewed his grandfather as a monster, only to find that he had grown up to become the one thing he feared the most. I'm not going to support myself. Read his autobiography.
