I think this represents how he is talking to his own heart in the 3rd person, as if it is a seperate entity to his head, because it is his head/mind that makes all the mistakes while it is his heart that has to deal with the pain and repercussions.

I think this song is totally about personal interpretation, however after having listened to this song for several years believing that he is apologising to a partner or ex partner for the pain he has caused, i think he is actually singing from his head to his heart and actually he is apologising to his heart for the pain he causes himself through his inability to be the person he think he is.

I totally agree about the Tyrion Lannister explaination. The song is written from the perspective of his brother Jaime, though (he's the lyrical ego). Duh. Makes perfect sense.

i think this song is about a person who made a choice or choices that ended really affecting or hurting some one they cared about. "it was not your choice but mine and was your heart on the line"
