I think that he's singing to his grandma because he wouldn't want his grandma to know how he had depression an that whole thing with drugs and alcohol he's kinda saying how he misses her

With all that Gerard and the guys have stated about the making of this album, or DURING the making of it, I find this and several other songs on this album to be very revealing.
The thought that this album is all about "the patient" is very romantic, isn't it? And it's obviously working for them. But the truth is, most of these songs are much more personal and closer to them than you may can imagine.
Several major incidents happened during the time of preparing and making this piece of work (their best so far). Do you honestly believe they couldn't bring those things with them into these songs?
These incidents could be seen as terminal conditions, or definitely, inevitable conclusions to certain things in their lives. There is a strong tone of reluctant acceptance throught this album.
Think about what they have talked about in some of the interviews they've given and then you'll start seeing these songs totally opposite of what you were originally told to see them as. -
I think that he is talking about a relationship that he was in where maybe the relationship stopped working. Maybe there was cheating involved or something hence the line "there's things that I have done, you never, should ever know." But even though the relationship was dissolving, he couldn't see himself living without that person.

I think in this song Gerard is saying that he can't live without someone like he's feeding on them and without them he dies.
