It was a hate message to paparazzi who Brian calls the scum of the earth.

It was about Brian May's affair with an actress but in terms of Freddie, it was more to do with rumours of him suffering from AIDS rather than his sexual orientation and that the press kept hounding him after his appearance changed over the course of the years between 1985 and 1989 when The Miracle was released.

The meaning is twofold, when this song was written the tabloids were having a field day reporting on rumors of Freddie Mercury's homosexuality, as well as Brian May's recent extramarital affair with an actress. The song was a collaboration between the two essentially telling the tabloids to back off, and beseching those in the know to keep quiet.
