I think he basically is talking about someone who isn't really showing their feelings and he can't tell if they have feelings for him or not and he's having a hard time debating whether or not to tell them. "You make it hard to smile" means when they arent showing any emotion, he's upset and "you make it hard to breathe" means that he's so blown away by them it takes his breath away. I could be way off, but that's just what I think.
