It's about empathy ("i know you're sick and alone") with, and trying to overcome depression. He bleeds (hurts) with the others who have it - the mothers and suckers. Its "all a/the rage" its common, almost a fashionable ailment which imprisons " all a cage".
He wonders why they have it because they are "average with simple minds and eyes and lungs and skin" (human).
Hes feeling it tonight and thinks of suicide - "Im giving blood tonight Im feeling just like I could give it all.." -
Probably something to do with shooting drugs.
