    He is reaching out to someone who is lost or hurting he cares about, letting them know they are not alone and are safe under his protection.
    I find this could be more specifically a parent calling out to their grown child who has drifted away from them.

    When I listen to the lyrics in this song I hear it as my higher self, a higher being talking, my instinct.

    Your inner voice. It will always protect you and keep you safe. You just have to learn to listen to it.

    Matt is singing about how if anything goes wrong, if you are lost in the darkness, to be brave because he will come to help. When you loose all your strength and you feel that your life has slipped out of hand you can follow him because he will not desert you. Even if "your fire has died out and they have left you for dead" you can follow him. He will keep you safe from anything and protect you. He will not hurt you so you can trust him.

    (Sorry if my sentences are a bit generic I listen to the song as I write the meaning)

    To summarize: You can follow Matt because he will always be there for you, even in your darkest of times. You may be hurt but he can, and will, protect you.
