I think this song talks about a girl that cuts herself. But only he knows that she is a cutter. She cuts herself because she feels alone. In School or wherever she is no one notice that she is in hurt because maybe she always smiles and seems so happy.
"Sometimes it all gets a Little too much" says, that even through she gets through a hard time she can make it and someday will be easier. He want to help her, but she doesn't want any help. When she's down she tells herself that she can get out of there and when she lose her Focus and don't now what do to do he is there for her.
