He's talking about a girl who he loved dearly but she clearly felt their relationship was over but couldn't bring herself to end it so he ends the relationship instead,"And I'll take the blame if its for your sake" he knows that she doesnt want to be the one to break up with him so he does it to save her the trouble. In the music video you can see them happy and in love, a contrast with the lyrics but this fits too because they must have tried to pretend as though things were as great as they were in the beginning but too many things must have changed and made her a stranger to him, "I can't sleep beside a stranger" and "We tried our best to find us, but there are no lights to guide us."

I think that the meaning is clear at the beginning.
"Tell me the wars you're fighting, behind that smile you're hiding"
Obvious- she is pretending to be fine when he knows she isn't.
"All the things I know you want to say"
She wants to tell him things, but 'can't.'
"We tried our best to find us, but there are no lights to guide us."
Because she's being closed they don't know how to act with each other. Things weren't as easy as they were in the beginning. They're trying to go back to what they once had, but can't.
"I can't sleep beside a stranger now."
He feels as if he doesn't know her.
"Piece by piece we fall apart, with every beat slow down my heart"
He's unhappy that their relationship is falling apart, also obvious.

He has to make the first move.
"And I'll take the blame if it's for your sake"
To save their relationship he'll take the hit instead of her, and blame himself though they both know it was her.
"No turning back on what you can't save"
That they can't really go back to the relationship they used to have, but they can push past it. -
He's talking about a relationship that feels like it's over, but she isn't saying it. Nothing is like it was in the beginning, and he's saying that they both know it, but it's too hard to let go.
Nothing has to be over if love can be shown from both sides.
