The Avett Brothers: And it Spread Meaning
And it Spread Lyrics
Then you left and it was through
Then the frost started in
My toes and fingertips
And it spread
And it spread into my heart
And it spread
And it spread into my heart
Then for I don't know how long
I settled...

I agree with the above description where it is about lost love, depression and self harm. I couldn't quite figure out the las part about "coming back with a brand new face", but reading the above interpretation, I believe his thought that the singer found new love, the love being the same, but in a new person is spot on.
I also find double meaning in lot of there songs where it can be about drugs but also relatable to other parts of life. Their lyrics definitely sound like at least one of the writers is in recovery.

It's about heroin.

I think "you" is actually love. In the beginning of the song the one whith whom he associated love leaves, leaving him feeling alone, depressed, and in the verge of self harm. The loss effected everything and everyone in his life. Then new love comes, though not through the person he thought. He realizes love is not a person but both an action and a noun. Once he realizes this, his life is full again and his joy of finding love pours into the world around him. The "drug" metaphor is not new. It's been used by artists both past and present.
I hope this helps; although, I'm only saying what I feel is the meaning behind the song. -
Some of the lines make me think it MUST be about heroin. Others make me think it couldn't possibly be. I love it either way.

I'm sure this song either has to do with self harm or drugs, intravenous or otherwise.
I like to think it does cause thats why it holds so much meaning for me, heh.
