I think it's pretty clear it's about oral sex, the positioning , the grappling of the wrist, the suggestion of meaningless repetition of the same act that you both enjoy and don't enjoy. However, I also could understand how it could involve drugs--in fact, this could be expanded to any activity that is both intimate and outside the bounds of conventional society.

I think the song is about all those things. It's about how his trying to fill in the void with sex drugs and cutting.
It says, "all my life I've been searching for something something never comes never leads to nothing nothing satisfies but I'm getting close."
It also has sexual references.."And if you open up wide
And if you let me inside." also it has references to cutting, " Calm down, don’t you resist /Your such a delicate wrist" and, " Another reason to bleed/One by one, hidden up my sleeve" meaning the cuts.
So my guess is that it's about all those things, but I could be wrong. :] I'm not quite sure. -

I think it's about self-harm because
"It gives me just what I need
Another reason to bleed
One by one hidden up my sleeve" shows the relief gained from it, the blood, and the fact that people who do it hide it (up their sleeves).
Also "Done, done, on to the next one" shows the cycle of self-harm and refers to cut after cut.
That's just what I think. -
Dave Grohl said himself that this song was about cunninglingus. Listen to the lyrics and you will understand.

I think this song is partly about alcohol, maybe drugs. The line "I love it but I hate the taste" gives it away. It is also about how he gets angry sometimes while he's drunk/high."One by One" and "Done, done, on to the next one" also seems to refer to bottle after bottle, or shot after shot.
