What if he or his „female friend“ is in a partnership.
Holding fast to truth means here eventually: not making an affair with her. They know each other from the age of a boy.
So he will not tell her his love and this hurts him.
At the end he says: so we drink to youth (and not to love) and nothing will happen. He keeps what he had as a boy... -

This to me is saying that love hurts like hell sometimes. (especially if your so in love that you won't give up.)It's saying that despite how much it hurts to stay with his love he can't walk away. He feels he couldn't bear to survive without it so he stays and bears the pain it brings with it.

When he says "should I surface this one man submarine", he is asking himself if he should allow himself to feel love again - a submarine was used in war to evade radar, so by referring to himself as a submarine he is saying he has been keeping himself from feeling love. Love is the radar, and his need for love is the submarine.
