Maybe it's about him knowing what's going to happen to him, and him saying, "That the hardest part of this is leaving you," meaning his loved ones. Even though he said "Know that I will never marry," and "I will not kiss you," which is probably what he would have done (or not done) if he realized what he was getting himself into.
This song makes me cry every time I hear it, because I think about Gerard dying... No. That'll never happen. XD -
This song is not about Gerard Is gay.
It has a storyline of the two demolition lovers (Bonnie and Clyde characters) getting in a huge gunfight in the center of the restaurant with these western criminal people. I guess the lovers escape from the restaurant. The police shoots down the two lovers, and arrests the western criminal people that survived in the gunfight. The criminals go to prison and the song Is also about prison and how bad It Is.
Its a long song to understand, but you have to read the lyrics. -
CORRECTION!
Gerard wrote this song BEFORE going on tour! He met McCracken TOURING for Revenge! so no this has nothing to do with a mini bar, a drunken night, or a party in a hotel room.

That is what Gerard says.
Obviously not about him directly MCR being notorious for heavy mediphors (Take Vampires for instance)
What isn't heavy in what Gerard said was
"Being very very bad" now us not knowing Gerard we don't know the exact occasion (my best guess just bad decisions)
bad decisions with consequences in the metaphor these consequences were "going to prison and getting fucked in the ass" don't think along those lines think more going someplace mentally and consequences to that mental place.
I could be completely wrong but I gave it a shot and now you know some facts -
Okay first of all this whole album is a conception album!
Three Cheers For Sweet Revenge is a conception album about 2 lovers that get stranded in the desert the girl is Helena and the man's name isn't mentioned. Okay so they get into a fight and he shoots Helena! Now to get her back because he didn't mean it and loves her desperately he makes a deal with the Devil that if he brings the souls of 1000 evil men he can have his beloved Helena back.
Now all you have to do is listen to the entire album and the story will make sense!.
Gerard said that (sorry I can't quote this) music has never told a REAL story before and he wanted to do that so Revenge is his if you ask me very comic booky version of that.
To clear the Gerard Way gay specualations he is Bisexual and did infact go with Bert Mccracken although they never reffered to it as dating but they were together.
But he is married now to Lyn-z and if you ask me no complaints... So yeah.
MCR is amazing btw. -
The song I think is this one person's story of getting arrested and everything they go through while in prison and the regret of being there.

I think that You Know What They Do... Is about an exploration of life, you know, MCR's life on the road. Its relevant to all the new bands coming out, like how they're all like wearing girlie pants and a crapload of makeup and long hair. You know how in prison, some guys are tough, but there is always one group of guys that band together and watch eachothers backs? well that's MCR.
