I think its about Vic's struggles with his depression and self harm, "counting the stars and scars" could be a reference to when he would cut as a teenager. "desperate times and desperate measures, Ive come so close, my hopes are severed by the downside." Could be about suicidal thoughts.

I recommend checking that out, as they do a very good job explaining it all. if I tried it would come out messy and confusing. the basic idea i think is someone he loves is a drug addict, and he still loves them and all. meanwhile his suicidal/self harm temptations and thoughts are getting to him.
