What Shawn is saying is that he has never been hurt but his hater brain made him think he was. His friend 5-starred him and he was sore for like months. He was roasted really hard in front of his squad and he was like"this hurts more than a knife." He was laying without breath because the pain and his friend gave him cpr and he woke up like thanks man.

I am a recovering heroin addict, and I have to say that this song was deliberately written to be interpreted multiple ways. To another heroin addict, the lyrics clearly indicate that the song is about heroin. However, to a non-addict, who wouldn't understand how we relate to the lyrics, would believe that the song sounds like a bad break up. A song about a break-up is much more socially acceptable in mainstream media than a song about heroin, so I have to say it's more of a subtle hint and left up to your own interpretation of the song.

A purely "heartbroken" explanation doesn't make that much sense. We typically don't refer to feelings of loss or rejection with cutting except a heart being cut into pieces. However, the fact he continually repeats, "gonna wind up dead," would indicate something more serious than hurt feelings.
I think the song refers to a person slitting their wrists due to a bad breakup. Typically survivors of this type of suicide attempt will end up with stitches from the laceration. He also states, "I'm left seeing red on my own," as a person committing suicide in this way would see a lot of blood and would do it alone. Furthermore, the phrase "going under" can refer to drowning or anesthesia, but in this context it would be "going under the knife". Also, "begging you to come help," refers to suicide being a "cry for help". -
It's definitely about heroin. Most heroin users refer to the drug as the thing they loved more then family, friends or a gf,bf. It's all they think about. So i know ill make it out alive if i quit calling u my lover, (stop taking heroin i qont die he is saying he knows he is stronger then the drug)move on. Needle and the thread...the needle and the band to wrap your arm. Bleed until I can't breathe (inject and hit a vein, u bleed and heroine users overdose and die mostly by respretory failure) Aching begging you to come help (withdrawl body aches begging heroin or a dealer to come help)

in my point of view I fell like the lyrics are saying. that he got his heart broken by a girl he really loved. he feels like its his fault that they broke up and he beats himself up over that. he beats his self up over it so much that he is needing stiches. he feels like he cant live without her kisses.

From My view it sounds like Shawn is singing about how a girl dumped him or it's about jealousy on how he's jealous of that girl who he really likes but she doesn't like her.

This song is about how when Shawn is broken up with a girl, he is heartbroken,and he can't get over the break up, hence "needle and the thread gotta get you out of my head, needle and the thread gonna wind up dead" he needs to get over the break up to STITCH his "wound" of the break up, or he will die.Evidence could be "I thought that I've been hurt before But no one's ever left me quite this sore" for the break up, as this is his worst one.
