This song is not about Kurt actually, I don't believe. In his 'back and forth' documentary he stated how people think it's about Kurt, but that it's actually about relationships in general; The instability of relationships and how you get involved with someone knowing how things will eventually end up because the person is damaged.

Dave said in an interview when his friend Jimmy od'd it was the most tragic thing that ever happened to him. Jimmy's death was at the most a year before they started working on the album

Its about kurt. In back and forth (movie not song)butch vig talks about the song and how he thought dave wrote it about kurt. Following that statement Dave stated that it is about how he was frustrated with someone he was involved with and didnt know what he was going to do.

I believe he's talking about Kurt and his suicide. When he says he can't forgive him, Dave is saying just what he is: he can't forgive him for taking his own life and bringing a halt to some of Dave's best years. He also goes on to say he "should have known" Kurt was on the border of life and death yet didn't do anything about it. Between repeating that over and over, Dave also tells of how he's still standing here [on this Earth].
In a way, Dave could even be talking about himself too and how he can't forgive himself for allowing things to go as they did back then. He feels guilt, even up to this day. -
I think it was someone after kurt. He said Ive been here before. And you can still love someone but still be angry with them and resent their self destruction.
