I think he's a guy who happens to be drinking and doing some reflecting at the time and smoking something too -- maybe weed which may make him a bit paranoid and cross that with the ideas he's having of not being able to trust or not know who he can trust among all the "new" people around him in his life he realizes he's dealing with "trust issues." I happen to think it is a song about different types of trust "issues." Love this song by the way.
