I think it's more than that. In the begging of the song there is a robotic voice from the fone saying: "this is the building office for Try Medical Center calling for Jason Mraz."
My first thought was that he tried to commit suicide and/or literally almost died before telling her how he feels. -
Basically, its like lifehouse's "hanging by a moment". He's in love with a girl who is most likely one of his closest friends. He wants to tell her, but if she doesnt feel the same he "...could be risking it all..." So he keeps holding out, out of fear and its "killing him" we've all been there.

Pretty much he's saying that he's in love with this girl and he knows he's better for her than her boyfriend. At the beginning, he's dreaming of her actually seeing that he loves her. He's just going through all these questions and scenarios in his head, and he'll do anything for this girl, even if it kills him.
