This is is precisely based on Dexter. Moreso the novels then the TV series. Old Friend is a reference to Dexter's Dark Passanger that pushes him to kill(actually a demonic entity in the novels).

I believe this song is talking about getting revenge on someone whose hurt you or hurt someone you love. And they gotta sacrifice the consequences.

Meaning it's too late for say sorry for what you've done, it's too late. -
I feel like this is about Dexter (the tv show...NOT the cartoon). Dexter goes after the people who kill innocents. Same as how they show no mercy to them, neither does Dexter. Everything from his mother's blood "uniquely mixing" with his as a boy.
The obvious similarities are there, though I could of course be way off!
